Medical Foot Care Podiatry (“we”, “us”, “our”) is committed to protecting the privacy of our patients and website visitors. This policy explains how we collect, use, store and disclose your personal information, including health information, and how we comply with the Privacy Act 1988 (Cth), the Australian Privacy Principles (APPs) and the Health Records Act 2001 (Vic).

The information we collect

The information we collect depends on your relationship with us. It may include:

  • contact details such as your name, address, telephone number and email address;
  • your date of birth and emergency or next-of-kin contact;
  • Medicare, DVA, NDIS and private health fund details where relevant to your care or claim;
  • health and medical history relevant to your podiatry treatment, including clinical notes, assessment findings and referrals;
  • payment and billing information; and
  • information about how you use this website, collected through cookies and analytics.

How we collect information

Wherever practical, we collect personal information directly from you, in person at a clinic, over the telephone, through our online booking system, or via forms you complete. We may also collect information from your referring general practitioner or specialist, and from Medicare or your health fund where you have authorised this.

Why we collect and use your information

We collect and use your information to:

  • provide safe, appropriate podiatry care and manage your treatment;
  • schedule, confirm and manage appointments;
  • process payments and claims through HICAPS, Medicare, DVA and the NDIS;
  • communicate with you and, where appropriate, with other practitioners involved in your care;
  • meet our professional, legal and record-keeping obligations; and
  • improve our services and the operation of this website.

Disclosure of your information

We may disclose your information to other practitioners involved in your care, to Medicare, DVA, the NDIS and private health funds for claiming purposes, and to our trusted service providers, including our online booking provider and secure IT and records providers, who are bound by confidentiality obligations. We may also disclose information where required or authorised by law. We do not sell your personal information. Where any provider stores data outside Australia, we take reasonable steps to ensure it is handled consistently with the APPs.

Storing and securing your information

We hold records in physical and electronic form and take reasonable steps to protect them from misuse, loss and unauthorised access through access controls, secure systems and staff confidentiality obligations. We retain health records for the minimum periods required by law, generally at least seven years from your last visit for adults, and until age 25 for patients treated as children.

Accessing and correcting your information

You may request access to the personal information we hold about you, and ask us to correct it if it is inaccurate, out of date or incomplete. To protect your privacy we will verify your identity before acting on a request. We will respond within a reasonable time. In limited circumstances permitted under the Privacy Act we may decline a request, and if so we will explain why.

Cookies and website analytics

This website may use cookies and analytics tools to understand how visitors use the site so we can improve it. You can disable cookies through your browser settings, though some features may not work as intended. Our online booking is operated by a third-party provider with its own privacy policy, which applies when you make a booking.

Making a complaint

If you have a concern about how we have handled your information, please contact us first so we can try to resolve it. If you are not satisfied, you may contact the Office of the Australian Information Commissioner at oaic.gov.au, or, for Victorian health records, the Health Complaints Commissioner.
